Our family doctors offer comprehensive primary care to patients signed up in their practice.
Our clinic is located inside the Alpha Corporate Centre. There are multiple other medical services available in this centre, including:
• Medical laboratory (bloodwork, urine tests)
• Imaging (X-ray, Ultrasound, Bone density)
• Pharmacy
• Audiology
• Physiotherapy/Massage Therapy
• Pain Clinic
UNINSURED SERVICES
Not all services provided by doctors are OHIP covered. Below is a list of services that are not covered through OHIP. These services require private payment and are the responsibility of the patient. The fees are assigned as per the recommendations of the Ontario Medical Association.
No show for regular appointment = $30
No show for full physical = $60
Doctor’s note/sick note = $20
Prescription for physio/massage/chiropractic services, orthotics = $20
Lost prescriptions/requisitions = $20
Simple form = $30
Complex form = $50-85
Third party full physicals = $125
Travel medicine appointment = $60
Uninsured vaccine administration = $15
TB skin test = $20 per step ($30 for form)
Transfer of medical records
paper = $30 for first 20 pages with an additional $0.25 per page thereafter
digital copy on CD = $40
